Rewards for Healthy Smiles

Your scheduled dental checkups are earning 250 points each calendar year for you. It is for your healthy smile.

– Step up Challenge

A healthy walking program that allows you to rack up 500 points equivalent to . You must keep track and log the walking habits of you and your family for the course of three or nine week. You can check with the program overview for the complete list of rules and regulations.

– Assessments for your CAN and HRA

Earn an amount worth or equivalent to 100 points per calendar year when you have accomplished the CAN and HRA of your health plan. Keep in mind that certain exclusions and restrictions may apply.

– Rewards for Healthy Pregnancy

Earn points worth 0 if you are pregnant and have joined in a prenatal program of your health plan. To qualify, you must be 18 years old or older and a current member of Medicaid. Received points are forwarded to the account carrier for participants 17 years old and below and dependents of their parents or their legal guardian’s account.

– Asthma Management

Refill for the prescribed asthma controller of your children and you are qualified to earn 750 points or worth 75$ every calendar year. To qualify, your child must be diagnosed with having persistent asthma and must be between the age of 5 and 17.

– Diabetes Management

Points worth are yours per calendar year every time you show up for a routine checkup at your physician and comply with the recommended tests.

– Bipolar Disorder Management

Refill your prescription medication for bipolar disorder and earn . You must be properly diagnosed and be at the age of 18 to 64 to qualify.

– Schizophrenia Management

As a participant diagnosed with schizophrenia and at the age of 18 and 64, you are entitled to receive . You will earn points every time you have your medication refills.

– Bone Density Test
